Comprehensive Hospice Care - Clinical and Integrative

Expert medical care, holistic therapies, and family support - all under one compassionate team.

What is hospice?

Support for the whole person, wherever home may be.

Hospice is specialized, team-based care for people facing a life-limiting illness, focused on comfort, dignity, and quality of life rather than treatment intended to cure the terminal illness. Care is designed to support the whole person - physically, emotionally, and spiritually - whether provided in the home or another care setting.

At Weavers of Care, we expand upon the traditional hospice model through an integrative, whole-person approach that incorporates holistic modalities for both patients and primary caregivers. Bereavement and ongoing family support remain an important part of that care.

Clear information for families

What hospice is - and isn't.

What hospice is

  • Specialized medical care focused on comfort, dignity, and quality of life.
  • Designed for certified terminally ill patients, typically with a life expectancy of six months or less.
  • Care provided where the patient lives - home, apartment, assisted living, or nursing facility.
  • Comfort-focused care managed by an interdisciplinary team.
  • Support for medical, emotional, and spiritual needs.
  • Support for family caregivers during the journey and beyond.
  • Not limited to six months; care continues while eligibility requirements are met.

Hospice helps people live each day as fully, comfortably, and meaningfully as possible.

What hospice is not

  • Hospice is not giving up.
  • Hospice does not hasten death.
  • Hospice is not only for the final days.
  • Hospice does not replace the caregiver - it supports them.

Hospice allows patients and families to shift focus from curing to connecting, healing, and making the most of the time that remains.

When to consider hospice

You may benefit from hospice if...

  • You feel overwhelmed caring for someone who is declining.
  • Treatments are no longer improving quality of life.
  • Your loved one has increasing symptoms or fatigue.
  • Your family wants guidance through important conversations and emotional closure.
  • You want remaining time to feel supported, intentional, and filled with dignity.
  • You are seeking compassionate support for both patient and caregiver.

Starting earlier allows for more support, better symptom management, and more meaningful time together.

What is hospice?

Hospice is a specialized, holistic model of care for people with terminal illnesses, focusing on comfort, dignity, and quality of life rather than curing the disease. It provides comprehensive medical, emotional, and spiritual support for patients and families, typically when a life expectancy of six months or less is anticipated.

Is hospice only for the final days of life?

Hospice can provide meaningful support much earlier in the journey through serious illness. Weavers of Care focuses on quality of life, dignity, and support for both patients and primary caregivers throughout the experience.

How much does hospice cost?

For eligible Medicare beneficiaries, the Medicare Hospice Benefit generally covers hospice services related to the terminal illness and related conditions. Limited cost-sharing may apply to certain outpatient medications and inpatient respite care. Medicaid and private insurance coverage varies, and our team can help verify benefits.

What can a home hospice agency offer you?

A home hospice agency can offer comfort-focused end-of-life care, palliative and symptom management support, grief and bereavement guidance, and comprehensive hospice and family support services.

What is the admission criteria for hospice care in Kansas?

To qualify, a physician must certify a terminal illness and a life expectancy of six months or less if the illness follows its expected course. This shifts the focus toward comfort care, symptom management, quality of life, and emotional support.

How do I start the hospice process?

Anyone can contact Weavers of Care to ask questions or request an evaluation—you do not need to wait for a physician to call us first. Our team reviews the patient's condition and goals, coordinates with the patient's physicians as appropriate, and completes the required hospice eligibility and certification process before admission.

Do I lose my doctor?

No. Choosing hospice does not mean giving up the doctor you know and trust. You may choose to have your physician continue participating in your care as your attending provider, working alongside the Weavers of Care hospice team.

Our goal is to add support—not take away trusted relationships. We coordinate closely with your healthcare providers so your care remains personal, connected, and centered on what matters to you.

Can I change my mind after choosing hospice?

Yes. Under Medicare rules, a beneficiary or representative may revoke the hospice election.

What levels of hospice care are available?

Medicare recognizes four levels of hospice care: Routine Home Care, Continuous Home Care, General Inpatient Care, and Inpatient Respite Care. The appropriate level is based on each patient's needs and may change as those needs change.

Your Weavers of Care team will help determine the appropriate level of care and explain what to expect.

How long does hospice stay with a patient?

A six-month prognosis does not mean hospice automatically ends after six months. Services may continue as long as the patient continues to meet Medicare hospice eligibility guidelines.

Does Medicare or Medicaid pay for hospice?

Yes, when the patient meets eligibility requirements. Many private insurance plans also provide hospice coverage.

How do I prepare my loved one for end-of-life care?

Choosing hospice does not mean giving up; it means prioritizing comfort, dignity, symptom management, and quality of life. Our interdisciplinary team supports patients and caregivers with nursing care, guidance, and holistic modalities.

What support services does Weavers of Care offer?

Our team supports the entire family with nursing, social work, caregiver guidance, emotional and spiritual support, communication assistance, advance care planning support, and holistic modalities designed to enhance comfort and connection.

Does Weavers of Care offer bereavement counseling?

Yes. Bereavement services and grief counseling continue after a patient's passing, helping families navigate grief, loss, and life transition with compassionate support.
