The people behind the care
Compassionate, skilled, and deeply committed to your family's well-being.
A circle of care around your family
Hospice care is strongest when clinical expertise, practical support, and human presence work together. Our care team brings those perspectives into one thoughtful plan for each patient and family.
Our nurses guide symptom management, medication support, care planning, and education with calm, experienced presence.
Hospice aides provide hands-on support with comfort, personal care, and the everyday details that help home feel more manageable.
Social workers, chaplains, and counselors help families navigate emotional, spiritual, and practical needs throughout the journey.
Holistic practitioners, bereavement counselors, and trained volunteers create space for comfort, connection, reflection, and meaning.
An interdisciplinary model
At Weavers of Care, compassionate hospice care is a team effort. Our interdisciplinary team brings together skilled nurses, hospice aides, social workers, chaplains, bereavement professionals, and trained volunteers, all working collaboratively to support the patient and the people who care for them.
That collaboration allows us to care for more than symptoms. We consider the whole person - physical comfort, emotional well-being, social needs, and spiritual concerns - while also making space for the caregiver’s needs, the family’s questions, and the moments that matter most.
Every patient’s hospice plan of care is guided by a Medicare-required interdisciplinary clinical team and individualized to their needs, goals, and preferences. When appropriate, Weavers of Care may extend that circle of support through integrative practitioners and additional supportive services designed to bring greater comfort, connection, and meaning to the hospice experience.
Together, we weave these different forms of care into one coordinated plan, centered on what matters most to each patient and family.
